Role Overview:

Support patient flow within Frailty Same Day Emergency Care.
Conduct assessments, re-enablement/rehabilitation, and discharge planning.
Work with inpatients in acute and general medicine settings.

Skills & Experience:

Band 6 physiotherapy experience in acute, general medicine, or rehab.
Able to manage same-day assessments and discharges efficiently.
HCPC registration required.

Working Hours:

37.5 hours per week.
7-day working pattern.
