Reablement Officer

Job Overview

Tripod Partners are currently recruiting for an experienced Reablement Officer to join Enfield Council within North London Social Services. This is an ongoing locum opportunity to cover a vacancy, initially offered as a 12-week placement with the likelihood of further 12-week extensions.

The successful Reablement Officer will undertake home-based initial and risk assessments, develop goal-focused reablement plans and monitor individuals’ progress to support independence and achieve positive outcomes. The role also involves medication management, care practice observations, competency assessments and the identification and escalation of safeguarding or other risks.

Key Responsibilities

• Complete timely home-based initial assessments and risk assessments.
• Develop person-centred, goal-focused reablement plans for support of up to six weeks.
• Review progress against agreed reablement goals and outcomes.
• Maintain accurate and comprehensive electronic care records.
• Complete medication assessments, MAR sheets and medication protocols.
• Undertake medication audits and carer competency observations.
• Observe care practice and identify areas requiring further support or intervention.
• Identify, manage and escalate risks and safeguarding concerns appropriately.
• Work collaboratively with individuals, carers, care providers and other professionals to support successful reablement outcomes.
• Undertake borough-wide home visits, working safely and effectively as a lone worker.
• Prioritise assessments, reviews and visits according to individual need and service requirements.
• Use approved digital and AI tools, including Microsoft Copilot and Beam, appropriately and in accordance with professional judgement and information governance requirements.

About You

We are looking for a confident and experienced Reablement Officer with a strong background supporting adults in home or community settings. You will have practical experience of assessment, risk management, reablement planning and medication management.

You will be organised and able to work independently, prioritising competing demands while maintaining high standards of record keeping and professional practice. You will also be confident undertaking lone home visits and responding appropriately to safeguarding concerns and changing circumstances.

Experience within reablement or domiciliary care would be particularly advantageous. You should also be comfortable using electronic care-record systems and approved digital tools to support effective social care practice.

Requirements

• NVQ/QCF Level 3 in Adult Social Care or equivalent.
• At least two years’ experience supporting adults in home or community settings.
• Experience within reablement and/or domiciliary care is preferred.
• Experience completing initial and risk assessments.
• Experience developing and reviewing reablement plans.
• Experience of medication management, including medication assessments, MAR sheets and protocols.
• Experience undertaking medication audits and/or carer competency observations.
• Experience using electronic care-record systems.
• Full UK driving licence and access to a vehicle for borough-wide home visits.
• Strong written and verbal English communication skills.
• Advanced Microsoft Word and Excel skills.
• Confidence using electronic systems and maintaining accurate records.
• Ability to work independently and prioritise effectively.
• Ability to undertake lone working safely.
• Knowledge of approved AI tools such as Microsoft Copilot and Beam, with appropriate professional judgement and understanding of information governance.

Working Hours & Rota

The role is offered at 35 hours per week, working from 9:15am to 5:15pm on a four-week rolling rota, including one weekend in four.

• Week 1: Tuesday, Wednesday, Thursday and Friday, 9:15am–5:15pm
• Week 2: Monday to Friday, 9:15am–5:15pm
• Week 3: Monday to Friday, 9:15am–5:15pm
• Week 4: Monday to Friday, plus Saturday and Sunday, 9:15am–5:15pm

Some flexibility will be required to respond to emergencies, rota changes or visits that overrun. With management agreement, hours may be adjusted on another day where required.
