Derbyshire County Council is seeking a committed and experienced Locum Social Worker to join the Adult Services team. This role involves providing statutory social work assessments, safeguarding, and care planning to adults with a range of needs, including older people, mental health, and physical disabilities. The successful candidate will work within a supportive team environment to promote independence and wellbeing.
Key Responsibilities:

Conduct Care Act 2014 assessments and reviews, developing person-centred care and support plans.

Lead safeguarding enquiries and risk assessments under the Care Act to protect adults at risk.

Manage a varied caseload of adults with complex needs, liaising with health, housing, and voluntary sector partners.

Support hospital discharge planning and transitions to community-based care where required.

Maintain clear, accurate case records and ensure compliance with council policies and statutory requirements.
