Undertake statutory responsibilities under the Care Act 2014, including assessments, support planning, reviews, safeguarding enquiries, and Mental Capacity Act duties.

Manage a varied caseload involving older people, adults with disabilities, mental health needs, and those requiring hospital discharge or reablement support.

Apply a strengths-based and person-centred approach, ensuring individuals are at the heart of care planning and decision-making.
