- For the Advanced Practitioner role, hold a small caseload while providing leadership, guidance, and management support to Occupational Therapists and Occupational Therapy Assistants.
- Conduct daily community visits to assess and review clients’ needs in their home environments, ensuring interventions are appropriate and effective.
- Work collaboratively with NHS colleagues, hospital teams, social workers, and community partners to achieve seamless care transitions.
- Maintain accurate, high-quality records and documentation in line with professional and council standards.
About You
You are an adaptable and proactive Occupational Therapist or Advanced Practitioner with a strong background in hospital discharge and reablement work. You thrive in fast-paced environments and are skilled in managing both clinical and leadership responsibilities. With excellent communication and problem-solving skills, you’ll be confident supporting individuals with a range of physical and functional needs while fostering collaboration within the multidisciplinary team.
Requirements
- Degree or diploma in Occupational Therapy and registration with the Health and Care Professions Council (HCPC).
- Previous experience within hospital discharge, reablement, or Discharge to Assess (D2A) services.
- Strong understanding of occupational therapy within adult social care and hospital-to-home pathways.
- For Advanced Practitioner roles: experience in staff supervision, leadership, and team coordination.
- Excellent assessment, clinical reasoning, and documentation skills.
- Ability to work on-site 4–5 days per week and conduct at least two visits daily.
- Previous NHS or local authority experience desirable.
