Occupational Therapist – Wiltshire Council, Monkton Park
Job Overview
We are seeking an experienced and compassionate Occupational Therapist to join Wiltshire Council’s Reablement Team based at Monkton Park. This locum role focuses on supporting individuals following hospital discharge, helping them rebuild confidence, restore independence, and reduce reliance on long-term care services.
You will play a vital role in delivering personalised, strengths-based assessments and reablement plans that empower people to live safely and independently. Working autonomously, yet fully supported by the Principal Occupational Therapist, you’ll provide professional guidance to colleagues, contribute to decision-making, and use your expertise to shape outcomes for residents across Wiltshire.
Key Responsibilities

- Conduct comprehensive, strengths-based assessments to identify individual goals, abilities, and support needs.
- Design and implement tailored reablement plans that promote independence and minimise the need for long-term care.
- Provide expert clinical advice and training to Reablement Team members to enhance service quality and consistency.
- Recommend and coordinate the provision of equipment and adaptations to enable safe and independent living.
- Work closely with health and social care professionals to ensure a holistic, joined-up approach to reablement.
- Maintain accurate documentation, ensuring all records, assessments, and reports are completed in line with Council and professional standards.
- Manage your own caseload and schedule with autonomy, balancing face-to-face visits and remote working effectively.
- Promote independence, confidence, and wellbeing for clients transitioning from hospital care to community living.
About You
You are a proactive and compassionate Occupational Therapist with a passion for helping people regain independence after hospitalisation. You combine strong clinical knowledge with creativity and problem-solving skills, and you are confident in making professional decisions and advising colleagues. You take pride in delivering person-centred, evidence-based interventions that make a measurable difference to people’s lives.
Requirements

- Degree or Diploma in Occupational Therapy with current HCPC registration.
- Minimum of two years’ post-qualification experience in a relevant setting (reablement, community, or hospital).
- Strong understanding of reablement principles and strengths-based practice within social care.
- Valid UK driving licence and access to a vehicle for client visits.
- Excellent communication, organisation, and time management skills.
- Ability to work both independently and collaboratively within a multidisciplinary team.
- Comfortable with hybrid working – 50% of hours will involve face-to-face visits in the community.
Contract & Salary

- Position: Locum Occupational Therapist – Reablement Team
- Location: Wiltshire Council, Monkton Park
- Area: Wiltshire Social Services
- Pay rate: £36.54 per hour
- Hours: 37 hours per week (Monday – Friday, 09:00 – 17:00)
- Contract length: Ongoing
- Working pattern: Hybrid (50% face-to-face community visits)
