Occupational Therapist – Band 6 (Homefirst Service)
Job Overview
We are recruiting an experienced Band 6 Occupational Therapist to join the Sussex Community NHS Foundation Trust based at Southfield House. This locum role is within the Homefirst Service, supporting early discharge and community rehabilitation through rapid response and short-term intervention.
The successful candidate will play a vital role in enabling patients to return home safely, promoting independence, and reducing hospital readmissions through timely, holistic occupational therapy assessment and intervention.
Key Responsibilities
– Conduct comprehensive functional assessments in patients’ homes following discharge or referral.
– Develop and implement individualised treatment and rehabilitation plans promoting independence and safety.
– Provide recommendations for equipment, adaptations, and ongoing therapy support as needed.
– Work collaboratively within a multidisciplinary team to ensure coordinated and effective care delivery.
– Contribute to discharge planning and community support arrangements for patients leaving acute settings.
– Maintain accurate and timely documentation in line with professional and Trust standards.
– Participate in service development and audit initiatives to enhance quality and efficiency.
About You
You’ll be a motivated and adaptable Occupational Therapist with a solid background in community rehabilitation or hospital discharge planning. You’ll bring strong clinical reasoning, communication, and organisational skills, alongside the ability to work autonomously in a fast-paced environment.
Requirements
– Degree or diploma in Occupational Therapy (or equivalent)
– Current registration with the Health and Care Professions Council (HCPC)
– Post-qualification experience within NHS community or hospital settings (essential)
– Experience with discharge planning, equipment provision, and functional assessments
– Strong communication and time management skills
– Car driver with access to own vehicle (desirable for community work)
