About the Role
Liverpool City Council is seeking a dedicated Reablement Assistant to support adults in improving and maintaining independence within their own homes and community settings. You will work as part of an integrated health and social care team, delivering personalised, strengths-based support aligned with individual care plans.
Main Responsibilities
- Meet the physical, social and emotional needs of service users in line with their care plans, promoting independence and enhancing daily living skills.
- Assist service users with tasks identified within their support plans.
- Support individuals to develop, improve and sustain everyday living activities, using appropriate equipment where required.
- Liaise effectively with other health and social care professionals to minimise risk and enable individuals to remain safely in their homes.
- Work as part of an integrated multidisciplinary team.
- Help service users follow their person-centred support plans and participate in assessments and reviews.
- Promote best practice in care, including dignity, respect, empowerment and cultural sensitivity.
- Maintain accurate records and report changes in service user needs to the Line Manager.
- Attend supervision sessions and work collaboratively with the wider team.
- Support and engage service users in social, recreational and wellbeing activities.
- Adapt flexibly to meet the evolving needs of the service.
- Work in line with City Council policies, including equality, safeguarding and anti-discriminatory practice.
Working Hours
- Monday to Friday, 09:00 – 16:00.
