Reablement Team Manager

Brief Job Overview

Tripod Partners are recruiting an experienced Reablement Team Manager for a locum opportunity with the Yorkshire Collaboration – Wakefield Council. Working within Yorkshire Social Services, you will lead a dynamic Reablement Service that supports older people to maximise their independence and safely remain within their own homes through high-quality, person-centred interventions.

This is an excellent leadership opportunity for an experienced manager with a background in Adult Social Care, Reablement, or Community Services. You will provide operational leadership to a multidisciplinary team, ensuring statutory responsibilities are met while driving service quality, performance, and positive outcomes for individuals transitioning from hospital or community settings.

Key Responsibilities

  • Provide day-to-day operational leadership for the Reablement Service, ensuring the delivery of high-quality, flexible, and responsive support.
  • Lead an integrated service working alongside health and social care professionals to promote independence and reduce long-term care needs.
  • Ensure referrals received from hospitals, community services, and primary care are managed effectively and delivered through person-centred care planning.
  • Manage, supervise, and support a team of Case Managers, including performance management, supervision, appraisal, and professional development.
  • Ensure statutory duties and Adult Social Care responsibilities are fulfilled in accordance with relevant legislation and best practice.
  • Monitor service performance, quality standards, and key performance indicators, implementing improvements where required.
  • Provide leadership in the delivery of strengths-based assessments and reablement interventions that maximise independence and wellbeing.
  • Work collaboratively with Integrated Care Teams, NHS partners, community organisations, and other stakeholders to coordinate effective care pathways.
  • Ensure services comply with relevant legislation, national guidance, local policies, and safeguarding procedures.
  • Contribute to the wider management and continuous development of Fieldwork Services.
  • Promote a positive team culture focused on continuous improvement, innovation, and excellent customer outcomes.

About You

You will be an experienced manager with a strong background in Adult Social Care, Reablement, or Community Services. You will have proven experience leading operational teams, managing performance, and delivering high-quality services that promote independence and person-centred care.

You will possess excellent leadership, communication, organisational, and partnership-working skills, together with a sound understanding of Adult Social Care legislation and integrated service delivery.

Requirements

  • Significant management experience within Adult Social Care, Reablement, Community Services, or a related setting.
  • Experience leading multidisciplinary teams and managing operational performance.
  • Strong knowledge of Adult Social Care legislation, safeguarding, and relevant statutory responsibilities.
  • Experience working collaboratively with Integrated Care Teams, NHS services, and partner organisations.
  • Excellent leadership, communication, coaching, and people management skills.
  • Ability to manage service delivery, quality assurance, and performance targets.
  • Strong organisational skills with the ability to prioritise competing demands.
  • Commitment to delivering high-quality, person-centred services that maximise independence.
