Undertake complex Moving and Handling assessments within service users’ own homes
Lead on Major Adaptations under the Disabled Facilities Grant (DFG) process
Assess individuals’ functional abilities using a strengths-based approach
Develop and implement person-centred intervention plans in partnership with clients
Enable people to live well within their communities through advice, therapeutic interventions, equipment and adaptations
