Hertfordshire County Council is seeking an experienced Community Care Officer / Occupational Therapy Assistant to join its Enablement Team in Apsley. The role focuses on proactive prevention and enablement, supporting individuals to achieve their identified goals within their own homes.

The successful candidate will work closely with commissioned care providers to help individuals develop skills, build confidence and increase their independence. Key Responsibilities

  • Provide proactive prevention and enablement support to individuals within their own homes.
  • Support people to work towards and achieve their identified goals.
  • Promote independence, confidence and the development of practical skills.
  • Work closely with commissioned providers to support effective enablement and reablement outcomes.
  • Encourage individuals to maximise their independence and participate in everyday activities.
  • Work collaboratively with colleagues and care providers to ensure support is focused on individual goals.
  • Monitor progress towards identified goals and contribute to appropriate reviews.
  • Provide person-centred support that promotes independence and reduces reliance on ongoing care where appropriate.

About You

We are looking for a motivated Community Care Officer or Occupational Therapy Assistant with experience supporting people in community or home-based settings. You will have a proactive and encouraging approach, with a strong focus on enablement, reablement and helping individuals develop their skills and independence.

You will be comfortable working closely with commissioned care providers and individuals in their own homes, supporting people to achieve meaningful goals while working as part of a wider service.

Requirements

  • Relevant experience as an Occupational Therapy Assistant, Community Care Officer or in a similar community-based role.
  • Experience supporting people within their own homes or community settings.
  • Understanding of enablement, prevention and/or reablement approaches.
  • Ability to support individuals to identify and work towards personal goals.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with commissioned care providers and colleagues.
  • Proactive, person-centred and outcome-focused approach.
  • Ability to work flexibly within the service’s core operating hours.

  • Weekend Working: This is not a 7-day service; weekend work requests will not be considered
