Social Care Assessor – Integrated Learning Disabilities Service (Transitions)

Job Overview

We are recruiting an enthusiastic Social Care Assessor to join an Integrated Learning Disabilities Service (ILDS) within North London Social Services. This rewarding locum opportunity focuses on supporting young people with learning disabilities as they transition from Children’s Services into Adult Social Care.

Working alongside the Senior Practitioner leading the Transition Project, you will play a key role in delivering strengths-based, person-centred assessments, developing support plans, reviewing care packages, and helping young people achieve positive outcomes as they move into adulthood. You will work within a highly collaborative, multidisciplinary team committed to promoting independence, wellbeing and choice.

Key Responsibilities

  • Undertake person-centred assessments for young people with learning disabilities transitioning into Adult Social Care.
  • Develop and review strengths-based support plans and care packages.
  • Work closely with young people, families, carers, schools, health professionals and partner agencies.
  • Support successful transition planning to maximise independence and positive life outcomes.
  • Participate in safeguarding activities in line with statutory responsibilities.
  • Contribute to the team’s duty function when required.
  • Maintain accurate case records and ensure assessments and reviews are completed within agreed timescales.
  • Promote person-centred practice and empower individuals to live as independently as possible.
  • Work collaboratively within a fully integrated multidisciplinary learning disabilities service.

About You

You will be passionate about supporting people with learning disabilities and their families, with a strong commitment to delivering high-quality, person-centred social care. You will have excellent communication and partnership-working skills, be organised in managing your workload, and enjoy working as part of a multidisciplinary team.

Requirements

  • Experience undertaking social care assessments within Adult Social Care.
  • Knowledge of working with people with learning disabilities and transition services.
  • Experience developing person-centred support plans and reviewing care packages.
  • Understanding of safeguarding legislation and best practice.
  • Ability to work collaboratively with families, education providers, health professionals and partner agencies.
  • Strong assessment, communication and report-writing skills.
  • Ability to prioritise workload and work independently while contributing effectively within a multidisciplinary team.
  • Commitment to strengths-based practice and promoting independence and wellbeing.

Contract & Salary

  • Role: Social Care Assessor – Integrated Learning Disabilities Service
  • Area: North London Social Services
  • Job Type: Locum
  • Contract: Ongoing (initial 3-month assignment with potential for extension or longer-term fixed-term opportunity, subject to service needs).
  • Salary: £23.87 per hour.
  • Working Pattern: Hybrid working – 3 days office-based and 2 days working from home.
  • Wednesday is a mandatory office day.
