Reablement Worker

Brief Job Overview

Tripod Partners are recruiting three Reablement Workers for locum roles within North London Social Services. These positions support adults within the community following illness or hospital discharge, helping individuals regain confidence, skills, and independence within their own homes.

This is a short-term, reablement-focused role aimed at promoting independence and reducing long-term dependency on care services. Reablement Workers will work closely with Occupational Therapists, Assistant Occupational Therapists, and wider multidisciplinary teams to support positive outcomes for service users.

Key Responsibilities

• Provide short-term enabling support to adults within their own homes.
• Support individuals following illness, injury, or hospital discharge.
• Promote independence and confidence in daily living activities.
• Assist service users to achieve reablement goals and maximise independence.
• Work collaboratively with Occupational Therapists, Assistant Occupational Therapists, and wider teams.
• Help identify appropriate levels of ongoing support where required.
• Maintain accurate records in line with service policies and procedures.
• Adapt to changing service demands and support rota coverage where needed.

About You

You will have experience supporting adults within 
community care, reablement, or domiciliary care services. You will demonstrate a 
person-centred and enabling approach, with a strong commitment to promoting independence and positive outcomes.

You will be reliable, adaptable, and confident supporting individuals with personal care and daily living 
activities within community settings.

Requirements

• At least 2 years’ experience supporting adults within their own homes.
• Experience providing personal care and support with daily living tasks.
• Understanding of reablement and strengths-based approaches.
• Full UK driving licence and access to a car or suitable vehicle (essential).
• Flexible approach to working patterns and service demands.
• Care Certificate or Level 2 NVQ/QCF in Care desirable.
• Strong communication and teamwork skills.

Shift Patterns

• Cluster 1: 20 hours over a 2-week rota
• Cluster 2: 36 hours over a 2-week rota
• Cluster 3: 8 hours per week
• Shift Times: 5:00 pm – 9:00 pm
