Brief Job Overview
Tripod Partners are recruiting a Social Care Assistant for a locum role within South East London Social Services. This position focuses on supporting adults and older people with a range of needs, including physical disabilities, mental health conditions, and learning disabilities, helping them maintain independence and wellbeing within the community.
The Social Care Assistant will undertake strengths-based assessments, support planning, and reviews, working closely with service users, carers, and multidisciplinary teams. The role involves promoting independence, facilitating access to appropriate services, and supporting individuals to remain in their own homes or transition to suitable accommodation where required.
Key Responsibilities
- Carry out strengths-based assessments of service users and their carers in line with the Care Act and eligibility criteria.
- Develop and implement person-centred support plans that promote independence and wellbeing.
- Manage a caseload of varying complexity with support from senior practitioners.
- Assess activities of daily living and arrange appropriate solutions such as assistive equipment and minor adaptations.
- Undertake risk assessments, including responding to safeguarding concerns with support from senior staff.
- Support service users in accessing services, including direct payments and community resources.
- Conduct Assistive Technology assessments and organise provision of suitable equipment.
- Contribute to Continuing Healthcare checklists as part of a multidisciplinary team.
- Support individuals with rehabilitation and ongoing care needs.
- Maintain accurate and timely records in line with policies and procedures.
- Work collaboratively with health, social care, and external partners to ensure coordinated support.

You will have experience working within social care services, supporting individuals with a range of needs including physical disabilities, mental health conditions, or learning disabilities. You will be confident undertaking assessments, supporting care planning, and working collaboratively with multidisciplinary teams.
You will demonstrate strong communication and organisational skills, along with a person-centred approach to supporting individuals and their carers to achieve positive outcomes and maintain independence.
Requirements
- Experience working within social care or a related setting.
- Understanding of the Care Act and strengths-based practice.
- Ability to undertake assessments and support planning for adults and carers.
- Experience managing a caseload with varying levels of complexity.
- Knowledge of safeguarding procedures and risk assessment.
- Strong communication, organisational, and IT skills.
- Ability to work collaboratively within multidisciplinary teams.
