Service: Home Adaptations to Independent Living (HAIL) Team

Job Details:

•  
  • Case Worker required to provide cover within the Home Adaptations to Independent Living (HAIL) Team
     

  • Support delivery of major home adaptations based on Occupational Therapist recommendations
     

  • Confirm Disabled Facilities Grant (DFG) eligibility and determine any client financial contributions
     

  • Complete Disabled Facilities Grant application forms and manage grant processes
     

  • Provide advice to Occupational Therapists considering own scheme adaptations
     

  • Oversee equipment adaptations including stairlifts and ceiling track hoists
     

  • Progress Warm and Healthy Homes grants and small repairs
     

  • Undertake visits across the borough as part of the role
     

  • Work within a small, high-performing and nationally recognised team

   

  Skills/Experience Required:

   

  •  
    • Good knowledge of Disabled Facilities Grant legislation and grants processes
       

    • Strong organisational and customer care skills
       

    • Flexible approach and ability to manage multiple cases
       

    • Experience within social care or home adaptations (suitable for experienced Occupational Therapy Assistant)

     

    Working Pattern: Hybrid – minimum 2 days per week in the office and borough visits, with home working for the remaining days.
