Report to an Occupational Therapist within Reablement
Carry out strengths-based occupational therapy assessments and reviews for individuals receiving occupational therapy equipment and domiciliary care
Undertake assessments under relevant legislation, including the Care Act and the Housing Grants, Construction and Regeneration Act
Reduce handoffs and ensure environmental changes that support independence are reflected in care provision
Work in partnership with service users, carers, and a range of professionals to promote independence and wellbeing
Deliver occupational therapy interventions that support independence
Accurately record contact and case notes on appropriate IT systems
