Brief Job Overview / Introduction to Role
We are recruiting a Community Care Support Coordinator to join the Reablement Service in the London Borough of Kingston. This locum position is ongoing, offering a role in a service dedicated to helping individuals regain independence in their own homes. The Reablement Service is distinct from standard home care, focusing on supporting users to achieve personal goals and live more independently.
Key Responsibilities
– Support the Reablement Manager in delivering a high-quality Reablement service. – Work directly with service users and carers to set achievable goals and conduct regular goal reviews. – Carry out Reablement tasks that assist service users in maintaining and improving their independence. – Record and manage the progress of service users effectively. – Provide support to the Reablement Manager or Deputy Reablement Manager as needed in the operational running of the service.
About You
– You will have a commitment to promoting independence for service users. – Excellent communication skills are essential to engage effectively with service users, carers, and colleagues. – You are organised, reliable, and able to manage records and reporting efficiently. – Experience in social care or a similar setting is advantageous.
Requirements
– Experience working in a care or Reablement setting is preferred. – Ability to work collaboratively with colleagues and external partners. – Strong understanding of goal-setting and monitoring progress with service users. – Flexibility to work across the London Borough of Kingston as required.
Contract & Salary
– Location: London Borough of Kingston, South West London – Salary: £20.79 per hour (negotiable) – Job Type: Locum, ongoing contract
