Job details:  OTA required for Reablement team – community based, multidisciplinary team. The post holder would be required to visit customers at homes throughout the week, work closely with OTs, SWs, physios and enablers. Majority of our referrals come from hospitals, and, in this role, the post holder would be supporting individuals in regaining independence post admission or other health deterioration. The team are currently required to work from the office at least 2 x a week.  this is likely to increase to 3 days from October 2024. JD available on request:
To provide a specialist Occupational Therapy assessment and Adult Social Care service provision for adult residents with substantial physical disabilities in accordance with relevant legislation and department eligibility criteria. In conjunction with the service user formulate a plan to enhance their independence through provision of equipment or through home adaptations. The needs of the carer are also to be carefully considered, assessed and where eligible addressed. The Occupational Therapy Assistant will work under professional guidance, training and supervision of an Occupational Therapist, and where required, the OT lead.
