Shropshire Community Health NHS Trust is seeking an experienced Occupational Therapist – Band 6 to provide a high-quality, seamless Occupational Therapy service across acute or community hospital and community settings.

The successful Occupational Therapist will work as part of a cohesive multidisciplinary team, providing responsive and person-centred Occupational Therapy interventions that meet the needs of the local population. The role will require flexibility to travel across the local community service, including Bishops Castle and Ludlow, depending on service demand.

Key Responsibilities

• Provide a high-quality Occupational Therapy service across acute or community hospital and community settings.
• Support a seamless transition of Occupational Therapy care from hospital to community services.
• Complete appropriate Occupational Therapy assessments and interventions based on individual needs.
• Work collaboratively as part of a multidisciplinary team to support positive outcomes for patients.
• Provide responsive Occupational Therapy input according to the needs of the local population.
• Support patients to maximise their independence and achieve appropriate functional outcomes.
• Communicate effectively with patients, carers and other professionals involved in their care.
• Maintain accurate and timely clinical records in accordance with professional and service requirements.
• Work flexibly across Ludlow and Bishops Castle community settings as service demand requires.

About You

We are looking for a confident and experienced Band 6 Occupational Therapist who can work effectively across hospital and community settings. You will be able to manage your workload professionally while contributing positively to a multidisciplinary team.

You will have a flexible and responsive approach and be committed to providing high-quality, person-centred Occupational Therapy. As the role involves travel between community locations, you must be a confident driver with access to your own vehicle.

Requirements

• Relevant Occupational Therapy qualification.
• Current HCPC registration.
• Relevant Occupational Therapy experience at Band 6 level or equivalent.
• Experience working within acute, community hospital or community settings.
• Ability to work effectively as part of a multidisciplinary team.
• Strong communication, assessment and organisational skills.
• Flexible approach to working across different community locations.
• Full UK driving licence.
• Access to and ownership of a vehicle is essential.
