East London NHS Foundation Trust is seeking an experienced Community Occupational Therapist to join the Community Occupational Therapy Service in North Bedfordshire. The post holder will provide high-quality holistic assessments, advice, treatment and evaluation to clients within the community, supporting rehabilitation, self-management and independence.

The role will involve visiting clients in their own homes, residential and nursing homes, and rehabilitation bedded units. The successful Occupational Therapist will manage an autonomous caseload of approximately 20–25 cases and work as part of a multidisciplinary service to support positive outcomes for clients with a range of presentations, including complex physical, psychological and neurological conditions.

Key Responsibilities

• Complete comprehensive, holistic assessments of clients’ functional, physical and psychological needs.
• Assess, recommend and order appropriate equipment to support clients and their carers.
• Demonstrate and provide guidance on the use of equipment to relevant care agencies.
• Work towards rehabilitation, self-management and independence throughout the continuum of care.
• Complete appropriate onward referrals to other relevant services.
• Seek advice and support from senior colleagues and appropriate services where required.
• Support hospital discharge planning and intervention when senior Occupational Therapy support is required.
• Provide Occupational Therapy support to rehabilitation bedded units as required.
• Review cases appropriately and within agreed timescales.
• Provide guidance and support to junior staff members as required.
• Work autonomously with responsibility for managing your own caseload.
• Work collaboratively within a multidisciplinary community service to promote excellent client outcomes.
• Supervise, educate and assess students at graduate level, working with universities to support appropriate practice and teaching standards.
• Travel across Bedfordshire as service demand requires.

About You

We are looking for a confident and experienced Community Occupational Therapist with strong experience of working within community services. You will be comfortable working autonomously, managing a varied caseload and completing holistic assessments in clients’ homes and other community settings.

You will have strong clinical reasoning and assessment skills, with the ability to identify appropriate equipment and interventions that promote independence and support clients and carers. You will also be confident working as part of a multidisciplinary team and providing guidance to junior colleagues and students.

Requirements

• Relevant Occupational Therapy qualification.
• Current HCPC registration.
• Strong experience working as a Community Occupational Therapist.
• Experience completing holistic and multifactorial assessments.
• Experience assessing for and recommending appropriate equipment.
• Ability to manage an autonomous caseload of approximately 20–25 cases.
• Experience working collaboratively within multidisciplinary services.
• Ability to support rehabilitation, discharge and independence-focused interventions.
• Ability to provide guidance and support to junior staff and students.
• Full UK driving licence and access to a vehicle are essential.
• Ability to travel across Bedfordshire as required by the service.
