Brief Job Overview

Tripod Partners are recruiting an experienced Band 6 Falls Prevention Therapist (Occupational Therapist or Physiotherapist) for a locum opportunity with Central and North West London NHS Foundation Trust, based at Ealing Day Treatment Centre, Southall. Working within the West London NHS Falls Prevention Service, you will provide specialist assessment and evidence-based interventions to reduce falls risk, improve mobility, and help people maintain their independence within the community.

This is an excellent opportunity for an Occupational Therapist or Physiotherapist with experience in rehabilitation, frailty, community therapy, or falls prevention. You will work closely with experienced clinical leads as part of a supportive multidisciplinary team, delivering therapy in both individual and group settings while contributing to the ongoing development of the service.

Key Responsibilities

• Provide specialist Occupational Therapy or Physiotherapy assessment for patients referred to the Falls Prevention Service.
• Undertake holistic assessments to identify falls risks, functional limitations, and rehabilitation needs.
• Deliver evidence-based falls prevention interventions through individual treatment sessions and group programmes.
• Develop personalised rehabilitation plans that promote mobility, confidence, independence, and safe functional performance.
• Monitor patient progress and adapt treatment programmes to achieve the best possible outcomes.
• Work collaboratively with the Lead Occupational Therapist, Lead Physiotherapist, Service Manager, and wider multidisciplinary team.
• Provide education, advice, and self-management strategies to patients, carers, and families to reduce falls risk.
• Support the supervision, development, and training of Rehabilitation Assistants and other team members.
• Maintain accurate clinical records in accordance with HCPC standards, Trust policies, and professional guidelines.
• Contribute to service development and continuous improvement to ensure the Falls Prevention Service achieves its objectives.

About You

You will be a HCPC-registered Occupational Therapist or Physiotherapist with experience working within rehabilitation, community therapy, falls prevention, frailty, or older people’s services. You will possess excellent assessment, rehabilitation, and communication skills together with a passion for promoting independence and preventing avoidable falls.

You will enjoy working as part of a multidisciplinary team while managing your own caseload and supporting the development of colleagues within the service.

Requirements

• Degree or Diploma in Occupational Therapy or Physiotherapy.
• Current HCPC registration.
• Experience within rehabilitation, community therapy, falls prevention, frailty, or older people’s services.
• Strong assessment, clinical reasoning, and evidence-based rehabilitation skills.
• Experience delivering therapy in both individual and group settings.
• Excellent communication and multidisciplinary team-working skills.
• Ability to support the supervision and development of Rehabilitation Assistants and junior colleagues.
• Commitment to person-centred, evidence-based practice and continuous professional development.
