Tripod Partners are recruiting an experienced Occupational Therapist for a locum opportunity within North West London Social Services. Based at the Brent Civic Centre, this role sits within the Adult Social Care Reablement Team, supporting residents to regain independence following hospital discharge or referral through community pathways.
This is an excellent opportunity for an Occupational Therapist with Adult Social Care or community experience who is passionate about promoting independence and preventing long-term care dependency. Working within a supportive multidisciplinary team, you will deliver strengths-based assessments, person-centred interventions, and rehabilitation plans that enable residents to remain safely in their own homes.
Key Responsibilities
- Undertake comprehensive Occupational Therapy assessments for adults with a wide range of physical, functional, and rehabilitation needs.
- Deliver strengths-based interventions that maximise independence and promote recovery following hospital discharge or community referral.
- Develop person-centred rehabilitation and reablement plans tailored to individual goals and outcomes.
- Carry out home visits to assess functional ability, environmental risks, and support needs.
- Recommend and arrange appropriate equipment, assistive technology, and minor or major adaptations where required.
- Work collaboratively with Social Workers, Care Assessors, Reablement Enablers, health professionals, and hospital teams to coordinate effective care.
- Support safe hospital discharge and help prevent unnecessary hospital admissions or long-term care placements.
- Provide advice and education to residents, carers, and families to support independence and self-management.
- Maintain accurate, timely, and professional clinical records in accordance with HCPC and local authority standards.
- Contribute to the delivery of an integrated, high-quality reablement service across the borough.
About You
You will be an HCPC-registered Occupational Therapist with experience working within Adult Social Care, community rehabilitation, or reablement services. You will be confident managing a varied caseload, completing home-based assessments, and working collaboratively within a multidisciplinary environment to achieve positive outcomes for residents.
You will possess excellent assessment, communication, and clinical reasoning skills, together with a commitment to delivering person-centred, strengths-based practice.
Requirements
- Degree or Diploma in Occupational Therapy.
- Current HCPC registration.
- Experience within Adult Social Care, Reablement, Community Occupational Therapy, or Integrated Care services.
- Strong knowledge of strengths-based practice and promoting independence.
- Experience completing functional assessments, home visits, and rehabilitation planning.
- Experience recommending specialist equipment and environmental adaptations.
- Excellent communication, organisational, and multidisciplinary team-working skills.
- Ability to manage a varied community caseload and work proactively.
- Availability to work within the borough for a minimum of three days per week.
