Brief Job Overview

Tripod Partners are recruiting an experienced Occupational Therapist for a locum opportunity within East Sussex Social Services. Based at Firwood House, supporting services across Eastbourne, Hailsham, and Seaford, this role sits within the Joint Community Rehabilitation Team, delivering rehabilitation and reablement interventions that enable adults to remain independent within their own homes.

This is an excellent opportunity for an Occupational Therapist with community rehabilitation experience who enjoys working as part of a multidisciplinary team. You will manage a varied community caseload, providing comprehensive assessments, rehabilitation programmes, and person-centred interventions that promote recovery, independence, and safe hospital avoidance or discharge.

Key Responsibilities

  • Undertake comprehensive Occupational Therapy assessments within community settings.
  • Develop and implement person-centred rehabilitation programmes that maximise independence and functional ability.
  • Manage a varied caseload of adults requiring community rehabilitation and reablement support.
  • Assess functional, cognitive, and environmental needs to develop effective intervention plans.
  • Recommend appropriate equipment, assistive technology, and adaptations to support safe independent living.
  • Work collaboratively within the Joint Community Rehabilitation Team alongside health and social care professionals.
  • Support safe hospital discharge and admission avoidance through effective rehabilitation planning.
  • Provide education and advice to service users, carers, and families to promote ongoing independence.
  • Maintain accurate, timely, and professional clinical documentation in accordance with HCPC and organisational standards.
  • Contribute to the delivery of high-quality, person-centred community rehabilitation services.

About You

You will be an HCPC-registered Occupational Therapist with experience working within community rehabilitation services. You will be confident working independently across a community caseload while collaborating effectively with multidisciplinary colleagues to deliver excellent patient-centred outcomes.

You will possess excellent assessment, communication, and clinical reasoning skills, together with a passion for supporting people to regain independence and remain safely within their own homes.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Previous community Occupational Therapy experience.
  • Experience delivering rehabilitation and reablement interventions.
  • Strong assessment, rehabilitation, and discharge planning skills.
  • Ability to manage a community-based caseload independently.
  • Excellent communication, organisational, and multidisciplinary team-working skills.
  • Access to a car for community visits is essential.
