Occupational Therapist or Physiotherapist – Reablement at Home

Brief Job Overview

Tripod Partners are recruiting an experienced Occupational Therapist or Physiotherapist for an exciting locum opportunity within East London Social Services. Based with the London Borough of Hackney, this role is part of an innovative Reablement at Home pilot service designed to help adults regain independence and achieve therapy-led goals within their own homes.

This short-term, therapy-led service supports individuals through personalised interventions, typically lasting up to six weeks, with the aim of maximising independence and reducing the need for ongoing care. The successful candidate will play a key role in shaping the future of reablement services while working collaboratively within a multidisciplinary team to deliver outstanding, person-centred care.

Key Responsibilities

  • Undertake comprehensive community-based assessments for adults with permanent, complex, or substantial disabilities.
  • Complete assessments in accordance with the Care Act 2014 and the Housing Grants, Construction and Regeneration Act.
  • Develop and deliver therapy-led, outcome-focused reablement plans that promote independence and reduce reliance on long-term care.
  • Work as part of a multidisciplinary team to review existing care packages and identify opportunities to maximise independence.
  • Assess and recommend specialist equipment, adaptations, Telecare, and assistive technology.
  • Review support packages, equipment provision, and environmental adaptations to ensure they continue to meet assessed needs.
  • Work in partnership with service users, carers, healthcare professionals, and other stakeholders to achieve positive outcomes.
  • Provide supervision and support to Occupational Therapy Assistants and Occupational Therapy students where required.
  • Maintain accurate, timely, and legally compliant case records using appropriate recording systems.
  • Contribute to the ongoing development of the Reablement at Home pilot and wider Adult Social Care service improvements.

About You

You will be an experienced Occupational Therapist or Physiotherapist with a passion for community rehabilitation and enabling adults to remain independent within their own homes. You will have strong assessment and clinical reasoning skills, together with the ability to work autonomously while collaborating effectively within a multidisciplinary team.

You will be committed to strengths-based, person-centred practice and enjoy supporting innovative approaches that improve outcomes for individuals and communities.

Requirements

  • Degree or Diploma in Occupational Therapy or Physiotherapy.
  • Current HCPC registration.
  • Experience working within Adult Social Care, community rehabilitation, or reablement services.
  • Knowledge of the Care Act 2014 and the Housing Grants, Construction and Regeneration Act.
  • Experience undertaking community assessments and delivering strengths-based interventions.
  • Experience assessing and prescribing equipment, adaptations, Telecare, and assistive technology.
  • Ability to review care packages and promote independence through therapy-led interventions.
  • Excellent communication, assessment, and multidisciplinary team-working skills.
  • Experience supervising assistants or students is desirable.
