Based with the London Borough of Hounslow, this role is part of a specialist Double-Handed Care Project, working closely alongside the Adult Social Care Review Team to maximise independence and promote strengths-based practice.

This is an excellent opportunity to contribute to a service improvement initiative focused on reviewing existing double-handed care packages. You will use your clinical expertise to assess functional ability, identify opportunities to promote independence, and recommend safe, evidence-based alternatives through equipment, moving and handling solutions, rehabilitation, and reablement interventions where appropriate.

Key Responsibilities

  • Review existing double-handed care packages to determine whether care can be delivered more safely and effectively.
  • Complete functional assessments using a strengths-based, person-centred approach.
  • Recommend appropriate equipment, assistive technology, and moving and handling solutions to maximise independence.
  • Identify opportunities to reduce reliance on care packages through Occupational Therapy interventions where appropriate.
  • Work collaboratively with the Adult Social Care Review Team and multidisciplinary professionals.
  • Provide specialist advice to service users, carers, and colleagues regarding safe practice and risk management.
  • Maintain accurate, timely, and legally compliant clinical records and assessment documentation.
  • Ensure practice complies with the Care Act 2014, safeguarding responsibilities, and HCPC professional standards.
  • Contribute to service improvement by supporting positive outcomes and promoting best practice across Adult Social Care.

About You

You will be an experienced Occupational Therapist with a strong background in Adult Social Care, moving and handling, and complex functional assessments. You will be confident reviewing care packages, applying clinical reasoning, and identifying innovative solutions that promote independence while maintaining safety.

You will be an excellent communicator who enjoys collaborative working and is committed to delivering high-quality, person-centred Occupational Therapy services.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Significant experience within Adult Social Care or Community Occupational Therapy.
  • Experience undertaking complex functional assessments and care package reviews.
  • Strong knowledge of moving and handling principles and equipment provision.
  • Experience assessing and reviewing double-handed care packages is highly desirable.
