Occupational Therapist – Home First Team

Brief Job Overview

Tripod Partners are recruiting an experienced Occupational Therapist for a locum opportunity within Dorset Social Services. Based with Dorset Council, this role sits within the Home First Occupational Therapy Team, supporting adults to return home safely following hospital discharge and promoting recovery, independence, and wellbeing through reablement-focused interventions.

Due to increasing demand across hospital discharge and community services, the successful candidate will play a key role in delivering person-centred Occupational Therapy assessments and interventions. Working closely with health, social care, and community partners, you will help facilitate safe discharges, prevent unnecessary admissions, and support individuals to maximise their independence within their own homes.

Key Responsibilities

  • Undertake comprehensive Occupational Therapy assessments for adults with a range of physical, functional, and social care needs.
  • Support hospital discharge pathways through timely assessment and intervention.
  • Develop and implement active recovery and reablement plans that promote independence and reduce reliance on ongoing support.
  • Work collaboratively with health, social care, and community partners to achieve positive outcomes for individuals.
  • Assess for and recommend appropriate equipment, adaptations, and support services.
  • Provide advice and guidance to service users, carers, families, and professionals.
  • Manage a varied caseload across Dorset, undertaking community visits as required.
  • Apply evidence-based Occupational Therapy practice and relevant theoretical frameworks to assessments and interventions.
  • Maintain accurate, timely, and legally compliant case records.
  • Contribute to a collaborative county-wide service model, maintaining links across all operational clusters.

About You

You will be an experienced Occupational Therapist with a strong background in adult social care, hospital discharge, reablement, or community-based practice. You will be confident working independently while building effective relationships with health and social care partners.

You will have excellent assessment and problem-solving skills, alongside a commitment to supporting adults to achieve greater independence and improved quality of life.

Requirements

  • Degree in Occupational Therapy.
  • Current HCPC registration.
  • Minimum of 3 years’ post-qualification Occupational Therapy experience within the UK.
  • Experience within adult social care, hospital discharge, reablement, or community services.
  • Knowledge of relevant theoretical frameworks, practice models, and evidence-based interventions.
  • Understanding of psychological and human development theories and their application in practice.
  • Strong assessment, care planning, and case management skills.
  • Excellent communication and multidisciplinary partnership-working abilities.
  • Access to a car and willingness to travel across Dorset.
