Community Neuro Occupational Therapist – Band 6 (2 Roles)

Brief Job Overview

Tripod Partners are recruiting two experienced Band 6 Community Neuro Occupational Therapists for a locum opportunity within East London NHS Foundation Trust. Based in Newham, these roles sit within the specialist Community Neurology Service, providing rehabilitation and ongoing management for adults with neurological conditions, including stroke, within their own homes and community settings.

The service delivers specialist, coordinated rehabilitation aimed at maximising independence, reducing the impact of disability, preventing hospital admissions, and supporting safe transitions from hospital to home. The successful candidates will manage complex neurological caseloads as part of a highly skilled multidisciplinary team.

Key Responsibilities

  • Provide specialist, client-centred occupational therapy assessment and intervention for adults with neurological conditions.
  • Manage a complex community caseload independently, applying advanced clinical reasoning and evidence-based practice.
  • Assess functional abilities and develop personalised rehabilitation programmes to maximise independence and participation.
  • Support individuals with conditions including stroke and other neurological disabilities within their homes and community environments.
  • Formulate treatment plans designed to reduce the impact of disability and optimise quality of life.
  • Work autonomously while maintaining close collaboration with multidisciplinary colleagues.
  • Contribute to hospital discharge planning and support smooth transitions from inpatient services to community rehabilitation.
  • Liaise effectively with inpatient stroke teams and other healthcare professionals across the neurological pathway.
  • Provide supervision, teaching, support, and appraisal for Rehabilitation Support Workers and students on placement.
  • Maintain accurate and comprehensive clinical records in accordance with HCPC and NHS standards.

About You

You will be an experienced Occupational Therapist with a strong background in neurological rehabilitation, community services, or stroke rehabilitation. You will be confident managing complex caseloads independently and delivering high-quality, evidence-based interventions within community settings.

You will possess excellent clinical reasoning, communication, and multidisciplinary working skills, alongside a passion for supporting individuals to maximise their independence and achieve meaningful rehabilitation goals.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Experience working with adults with neurological conditions and/or stroke.
  • Community rehabilitation experience highly desirable.
  • Strong clinical reasoning and complex case management skills.
  • Experience delivering person-centred and evidence-based rehabilitation programmes.
  • Ability to work autonomously and prioritise a complex caseload.
  • Experience supervising, supporting, or mentoring staff and students.
  • Excellent communication and multidisciplinary team-working abilities.
