Community Occupational Therapist or Physiotherapist – Band 5/6

 

Brief Job Overview

 

Tripod Partners are recruiting a Community Occupational Therapist or Physiotherapist – Band 5/6 for a locum role within South West NHS. This position sits within an Intermediate Care Team, delivering therapy services to adults within their own homes.

 

The successful candidate will provide patient-centred, evidence-based rehabilitation to support both acute and long-term needs. Working as part of an integrated multidisciplinary team, the role focuses on supporting individuals with complex needs, including frailty, to improve independence and functional outcomes.

 

Key Responsibilities

 

  • Deliver occupational therapy or physiotherapy assessments and interventions within community settings.
  • Provide safe, effective, and patient-centred rehabilitation programmes.
  • Manage a caseload of adults, including those with complex and frailty-related needs.
  • Work autonomously while recognising when to seek guidance from senior staff.
  • Contribute to multidisciplinary team (MDT) working alongside nurses and allied health professionals.
  • Support both acute and long-term therapy interventions within the community.
  • Maintain accurate clinical documentation and records.
  • Apply evidence-based practice and reflect on clinical performance.

 

About You

 

You will be a qualified Occupational Therapist or Physiotherapist with recent 
experience in community settings, particularly working with frail or complex patients. 
You will demonstrate strong clinical reasoning skills and the ability to manage a caseload independently.

 

You will be confident working within a multidisciplinary team, delivering rehabilitation interventions 
that support independence and improve patient outcomes.

 

Requirements

 

  • Recognised qualification in Occupational Therapy or Physiotherapy.
  • Registration with the Health and Care Professions Council (HCPC).
  • Recent experience within community therapy services.
  • Experience managing complex or frail patients.
  • Ability to work autonomously and manage a caseload effectively.
  • Strong IT, communication, and organisational skills.
  • Full UK driving licence, access to a vehicle, and appropriate business insurance (essential).
  • Experience in moving and handling, including hoists and equipment (desirable).
  • Experience in postural management assessment (desirable).
