Brief Job Overview

 

Tripod Partners are recruiting an Occupational Therapist for a locum role within South West London Social Services. This position sits within a Reablement Service, supporting adults to regain independence, improve wellbeing, and reduce reliance on long-term care.

 

The Occupational Therapist will manage a caseload of adults aged 18+, delivering assessments, reviews, and interventions within community settings. The role focuses on strengths-based, person-centred practice, supporting individuals and their carers to maximise independence and achieve meaningful outcomes following illness, injury, or changes in ability.

 

Key Responsibilities

 

  • Carry out occupational therapy assessments, reviews, and ongoing support within reablement services.
  • Manage a caseload of adults requiring short-term support and rehabilitation.
  • Complete regular home visits and provide person-centred interventions.
  • Work collaboratively with care agencies, families, and carers to support independence and wellbeing.
  • Assess and recommend equipment and minor adaptations to support daily living.
  • Undertake moving and handling risk assessments and develop Personal Handling Plans.
  • Complete Care Act assessments, carers assessments, and mental capacity assessments.
  • Maintain accurate and up-to-date case records using Mosaic and other systems.
  • Work within statutory frameworks including the Care Act 2014.

 

About You

 

You will be a qualified Occupational Therapist with experience working within adult social 
care or reablement services. You will demonstrate strong clinical reasoning and the ability to support 
individuals in achieving improved outcomes through strengths-based approaches.

 

You will be confident working independently, managing a varied caseload, and collaborating with 
multidisciplinary teams to deliver coordinated and effective care.

 

Requirements

 

  • Recognised qualification in Occupational Therapy.
  • Registration with the Health and Care Professions Council (HCPC).
  • Experience working within adult social care or community services.
  • Knowledge of relevant legislation including the Care Act 2014.
  • Experience in moving and handling assessments and equipment provision.
  • Strong report writing, documentation, and time management skills.
  • Experience working within multidisciplinary and partnership settings.
  • Previous reablement or local authority experience desirable.
