Reablement Occupational Therapist

Brief Job Overview

Tripod Partners are recruiting a Reablement Occupational Therapist for a locum role within East London Social Services. This position focuses on supporting individuals through reablement services, helping them regain independence and reduce reliance on long-term care.

The Occupational Therapist will manage a small caseload, working collaboratively with Reablement Officers, Social Workers, and health professionals to deliver assessments and interventions. The role involves a strengths-based, person-centred approach, supporting individuals within their homes and communities to achieve meaningful outcomes.

Key Responsibilities

• Carry out occupational therapy assessments within reablement services.
• Manage a caseload of approximately 4 cases per week.
• Undertake assessments including double-handed care reviews, moving and handling assessments, and adaptation specifications.
• Recommend and support provision of equipment and minor adaptations such as wet rooms and stairlifts.
• Work jointly with Reablement Officers, Social Workers, and health professionals.
• Support staff to work towards SMART goals and deliver effective interventions.
• Undertake joint visits and demonstrate techniques to support rehabilitation goals.
• Provide professional advice and guidance to colleagues and stakeholders.
• Maintain accurate records using social care electronic systems.
• Contribute to team meetings and support service capacity across reablement teams.

About You

You will be a qualified Occupational Therapist with experience working within 
reablement or rehabilitation services. You will demonstrate strong clinical reasoning and 
a person-centred approach, supporting individuals to maximise independence and wellbeing.

You will be confident working collaboratively with multidisciplinary teams and supporting colleagues to 
deliver effective, goal-focused interventions.

Requirements

• Recognised qualification in Occupational Therapy.
• Registration with the Health and Care Professions Council (HCPC).
• Experience working within reablement services.
• Experience with major adaptations and equipment provision.
• Experience undertaking moving and handling and double-handed care assessments.
• Ability to work collaboratively with multidisciplinary teams.
• Strong communication and organisational skills.
• Willingness to travel across the borough (car desirable but not essential).
