Job Title: Occupational Therapist (Multiple Roles)

Service: Reablement and Housing Services

Job Details:

• Multiple Occupational Therapists required to support Reablement and Housing services.
• Carry out occupational therapy assessments for adults with physical disabilities and complex needs.
• Support individuals to maximise independence and remain safely in their homes.
• Assess for equipment, environmental adaptations and housing solutions where required.
• Develop and implement person-centred intervention plans.
• Work collaboratively with multidisciplinary teams including social workers, housing teams and health professionals.
• Maintain accurate case records and documentation in line with service standards.

Skills / Experience:

• Qualified Occupational Therapist with HCPC registration.
• Experience working within adult social care.
• Experience in reablement and/or housing adaptations.
• Ability to manage a caseload independently.
• Strong communication and multidisciplinary working skills.
• Full UK driving licence and access to a vehicle (essential).

Working Hours: Full time or part time considered.
