Service: Adult Social Care – Major Adaptations
Job Details:
Occupational Therapist required to support Adult Social Care services focusing on major adaptations.
Carry out occupational therapy assessments for adults with physical disabilities and complex needs.
Assess and recommend major housing adaptations to support independence and safety at home.
Work closely with housing services, surveyors and multidisciplinary teams to implement adaptation solutions.
Support service users to remain safely in their homes and reduce the need for long-term care packages.
Maintain accurate case records and documentation in line with service standards.
Skills / Experience:
Qualified Occupational Therapist with HCPC registration.
Experience within Adult Social Care.
Experience assessing for major adaptations and housing solutions.
Knowledge of equipment provision and moving and handling.
Ability to manage a caseload independently.
Strong communication and multidisciplinary working skills.
