Job Title: Occupational Therapist

Salary: £35–38 per hour

Job Details:
Occupational Therapist required to provide an outcome-focused assessment service for adults with physical disabilities and age-related conditions.
Deliver a professional Occupational Therapy and Reablement service, including statutory social care responsibilities.
Carry out person-centred, strengths-based assessments to promote independence and recovery.
Work collaboratively with multidisciplinary teams and partner agencies to support individuals with complex needs.
Support safe and timely hospital discharges and step-down care in community settings.
Manage a varied caseload including learning disabilities, older adults, physical disabilities and mental health.
Maintain accurate case records and documentation in line with service requirements.
Skills / Experience:
BSc (Hons) or MSc in Occupational Therapy.
Current HCPC registration and RCOT membership.
Experience within a Local Authority setting.
Experience in hospital discharge and community-based work.
Knowledge of Mosaic case management system (desirable).
Experience working with a wide range of adult service users.
Strong communication and multidisciplinary working skills.
Full UK driving licence and access to a vehicle (essential).
Working Pattern: Hybrid – minimum 2–3 days per week in the office.
