Service: Transfer of Care Team – North Bedfordshire Therapy Service
Job Details:
Band 6 Occupational Therapist required to start ASAP within the Transfer of Care Team.
Support both acute and community settings to facilitate safe and timely discharges and improve patient flow.
Focus on Pathway 1 discharge planning and preventing unnecessary hospital admissions through urgent care response.
Provide support to Inpatient Rehabilitation Beds as part of the wider therapy service.
Deliver high quality multifactorial assessments, advice, treatment and evaluation for clients with diverse and complex needs.
Work with individuals presenting with a range of physical, psychological and neurological conditions.
Support rehabilitation, self-management and independence across the continuum of care.
Work as an autonomous practitioner in line with HCPC professional standards.
Manage an independent caseload and contribute to the delivery of an efficient integrated therapy service.
Supervise, educate and assess students to graduate level, working collaboratively with universities to maintain professional standards.
Travel across Bedfordshire as required to support service demand.
