Job Details:
Occupational Therapist required to provide a range of occupational therapy interventions for adults and children referred to the service, enabling individuals to remain in their own homes with optimum levels of independence in all aspects of daily living.
Key Responsibilities:
- Assess adults and children with disabilities and arrange provision of equipment and adaptations to maximise independence and safety, including major adaptations where required.
- Undertake complex moving and handling risk assessments and develop appropriate management plans.
- Actively involve service users and carers in assessments, support planning and accessing Direct Payments.
- Identify and recommend assistive technology solutions and provide demonstrations to ensure safe use.
- Maintain accurate and timely case records and review support plans to ensure ongoing effectiveness.
- Update systems to support service delivery and the collection of relevant statistical data.
- Provide supervision and professional guidance to Occupational Therapy Assistants and Social Workers where appropriate.
- Work collaboratively with health, social care and other agencies to support integrated service delivery.
- Support the Reablement service where demand requires.
- Participate in service development and ongoing professional development.
Skills / Experience:
- Degree or Diploma in Occupational Therapy.
- Current HCPC registration.
- Experience working in adult social care or adult health settings.
- Knowledge of safeguarding policies and procedures.
- Understanding of the Care Act 2014 and the wider health and social care agenda.
- Ability to undertake holistic therapy assessments and risk assessments.
- Ability to plan and evaluate care using sound professional judgement.
- Experience developing and implementing reablement goals and plans.
- Strong written and verbal communication skills.
- Ability to work collaboratively with professionals and partner agencies.
- Strong organisational skills with the ability to prioritise workload effectively.
- IT skills including use of spreadsheets, databases and email systems.
