Job Details:
Occupational Therapist required to join the Minor Aids & Adaptations Team
Work within a fast-paced environment where the team aims to complete assessments and cases within approximately 15 working days
Promote strengths-based practice to support independence, health and wellbeing
Carry out needs-led assessments in line with the Care Act for residents and their carers
Provide Occupational Therapy services to individuals with physical disabilities and complex needs, including those with mental health conditions
Work as part of a multidisciplinary team to assess needs and develop appropriate intervention plans
Use professional judgement within relevant legal and policy frameworks
Plan and implement interventions that support independence and reduce or delay the need for ongoing care
Promote cost-effective and efficient use of resources in assessment and intervention
Assess carers’ needs and provide support where eligible
