Job Details:
Community-based Occupational Therapist roles within an integrated Neurology and Stroke service
Undertake comprehensive assessments and deliver rehabilitation programmes for patients with stroke and neurological conditions
Manage a community caseload, providing goal-focused, person-centred interventions
Work collaboratively within a multidisciplinary team to support recovery and maximise independence
Contribute to discharge planning and ongoing community rehabilitation pathways
Maintain accurate documentation in line with NHS standards and service requirements
Skills/Experience Required:
Experience in stroke and/or neurology rehabilitation
Community experience beneficial
HCPC registration as an Occupational Therapist
Full driving licence and access to own vehicle essential for community visits
Working Hours: 37.5 hours per week. Weekday working only. Full time preferred; flexible arrangements will be considered.
