Job Title: Occupational Therapist (2 Roles)

Job details: Occupational Therapists required to support annual reviews and equipment assessments across the county. Full-time position with a minimum of 1 day per week office-based (additional office presence may be required depending on service needs).
Undertake annual reviews including bed leavers, bed raising, hoists and manual handling assessments
Provide assessment and recommendations to ensure safe and appropriate equipment provision
Carry out county-wide visits – driving licence essential
Claim mileage from office base to visits
Maintain accurate documentation and records in line with service requirements
Work collaboratively within Adult Social Care to support safe and effective outcomes
