Job details: Community Occupational Therapist required for a Discharge to Assess service.

•  
  • HCPC registration required
     

  • Community-based role – driving licence and access to a vehicle essential
     

  • Evaluate service users’ fitness for rehabilitation at the time of assessment
     

  • Assess, plan, implement and evaluate rehabilitation interventions
     

  • Undertake assessments and interventions across a range of community settings
     

  • Create goal-focused rehabilitation plans as part of the multidisciplinary care plan
     

  • EMIS experience essential

   

  Working hours:

   

  •  
    • 37.5 hours per week
       

    • Preferred: 3 long days (08.00 – 20.00)
       

    • Alternative: 4 days (08.00 – 18.00)
       

    • 1 weekend worked every 3–4 weeks
