Job details: Occupational Therapist or Physiotherapist required to start ASAP for Community Therapy Wards.
Provide therapy input within community-based hospital wards
Undertake mobility assessments and develop individualised exercise programmes
Support rehabilitation and reablement of patients
Contribute to effective discharge planning to support safe transitions from hospital
Provide and prescribe appropriate equipment, including manual handling equipment
Supervise and support junior members of staff
Skills & experience required:
HCPC registered Occupational Therapist or Physiotherapist
Experience working on hospital wards within a rehabilitation or reablement setting
Strong skills in mobility assessment, exercise prescription and discharge planning
Manual handling experience essential
Knowledge of common conditions including arthritis, heart disease, diabetes, dementia, MSK conditions, COPD, orthopaedics and falls
Car driver essential – role requires transporting items of equipment
