Allocated Occupational Therapist for adults and children with physical disabilities and learning disabilities, providing preventative support to promote independence
Responsible for assessing, developing, reviewing and monitoring individual care plans relating to adaptations, equipment, and moving and handling to support independent living
Work within a strengths-based model, focusing on prevention, promoting independence, and identifying solutions to enable citizens to remain in their own homes
Commission services for the provision of adaptations and equipment, enabling citizens and carers to achieve their full potential for independent living
Liaise with voluntary and statutory organisations at local and national level
Act as Duty Officer, providing consultation to internal care management teams, external health and social care professionals, and members of the public regarding disability, equipment solutions, and moving and handling
Screen referrals into the Occupational Therapy service
