Brief Job Overview

The Royal Borough of Kensington and Chelsea is recruiting an Independent Living Assessor to join their busy Information and Advice (Front Door) Team. This service is the first point of contact for Adult Social Care, supporting residents through advice, screening, and early intervention to promote independence and wellbeing.

This is a fast-paced role ideal for an enthusiastic practitioner who enjoys short-term, solution-focused work and managing a high volume of queries and referrals.

Key Responsibilities

  • Answer incoming calls and manage a range of Adult Social Care queries.
  • Screen and process referrals, including safeguarding concerns.
  • Provide appropriate signposting to internal and external services.
  • Support Social Work and Occupational Therapy teams with access to services, equipment, and minor adaptations.
  • Promote independence and wellbeing of residents across the borough.
  • Accurately document case actions, contacts, and assessments using Mosaic.
  • Work as part of a high-demand front door service handling urgent and varied enquiries.

About You

You will be confident working in a busy front-line environment, able to triage information quickly and provide clear advice and direction. You will be organised, customer-focused, and comfortable using case management systems.
 

Requirements

  • Experience working within Adult Social Care or a related front door / triage service.
  • Understanding of safeguarding processes and referral pathways.
  • Ability to provide information, advice, and signposting appropriately.
  • Experience supporting social care services, equipment provision, or minor adaptations (desirable).
  • Experience using Mosaic or similar case management systems.
  • Strong communication, organisational, and decision-making skills.

Working Pattern

  • Office presence required for 50% of the working week.
  • Full-time hours (standard local authority working pattern).
