Job Details

 

An Occupational Therapist is required to start ASAP to support the delivery of statutory Occupational Therapy services to adults with care and support needs. This role involves assessment, intervention, care planning and multi-agency working within a community social care setting. JD available on request.

 

  •  
  • Deliver statutory Occupational Therapy services to adults with care and support needs, their families and carers.
     

  • Undertake Occupational Therapy assessments, planning and reviews for services, equipment and adaptations (minor and major).
     

  • Provide professional advice, support and information in partnership with adults, families and carers.
     

  • Contribute to multi-agency planning, implementing actions and coordinating resources to meet identified needs.
     

  • Review and monitor services and resources, including safeguarding considerations.
     

  • Commit to safeguarding adults, respecting their views, wishes and outcomes.
     

  • Contribute to the ongoing development of the team and service, promoting anti-discriminatory practice and meeting agreed performance standards.
     

  • Participate in individual and group supervision, training and professional development activities.
     

  • Maintain accurate and timely records in line with Data Protection legislation and departmental policies.
     

  • Support service planning by contributing to business planning, service monitoring and performance evaluation.
     

  • Undertake direct work with individuals, families and groups, working collaboratively with other teams and services.
     

  • Work flexibly to meet statutory requirements and the needs of adults with care and support needs.

 

Skills & Experience

 

  •  
  • Qualified Occupational Therapist.
     

  • Community-based experience preferred.
     

  • Experience of working within Adult Social Care desirable.
     

  • Strong assessment, communication and organisational skills.
     

  • Access to a vehicle with business insurance required to undertake community visits.
