Occupational Therapist – Band 6 (Part Time, Locum)

Job Overview

We are recruiting a Band 6 Occupational Therapist to join Cwm Taf Morgannwg University Health Board, based at Dewi Sant Health Park, Pontypridd, within the South Wales NHS. This part-time locum role is required to start as soon as possible for an initial 6-week period to support the Hospital @Home Service. The service delivers clinic-based and home-based assessment and intervention aimed at admission avoidance for frailer older adults approaching crisis across the Merthyr and Rhondda Cynon Taf areas.

 

Key Responsibilities

  •   
    • Respond rapidly to referrals for patients at risk of hospital admission, providing same-day assessment where required.
       

    • Undertake home-based occupational therapy assessments to review the home environment and manage identified risks.
       

    • Provide crisis intervention, including urgent manual handling assessments and equipment provision.
       

    • Support admission avoidance through positive risk management and timely occupational therapy input.
       

    • Manage a caseload effectively, prioritising work in line with service demand and patient acuity.
       

    • Work closely with the multidisciplinary team, including Consultant Geriatricians, Advanced Nurse Practitioners, nurses, physiotherapists, and therapy technicians.
       

    • Contribute to MDT triage meetings to prioritise referrals and coordinate interventions.
       

    • Liaise with community services, local authority teams, and third-sector organisations to support onward rehabilitation, environmental adaptation, or longer-term needs.
       

    • Support therapy escalation for patients within the Hospital @Home and Bridging pathways.

     

    About You

    You are a confident and responsive Occupational Therapist with experience working with older adults in community or intermediate care settings. You bring strong assessment and clinical reasoning skills, are comfortable working under pressure, and are able to manage urgent referrals effectively. You value multidisciplinary working and are committed to supporting people to remain safely at home wherever possible.

    Requirements  

      • Degree or diploma in Occupational Therapy with current HCPC registration.
         

      • Experience working with older adults, ideally within community, admission avoidance, or intermediate care services.
         

      • Strong skills in functional assessment, risk management, and manual handling.
         

      • Ability to respond rapidly to urgent referrals and manage competing priorities.
         

      • Excellent communication, documentation, and multidisciplinary team-working skills.
