Job Overview
We are recruiting an experienced Band 6 Occupational Therapist to join the Intensive Rehabilitation & Therapy Service at NELFT, based at Grays Court Community Hospital within the North East London NHS. This community-focused locum role involves delivering high-quality occupational therapy assessment, treatment, and discharge planning for patients supported by a community rehabilitation team. The post suits an Occupational Therapist who is confident working autonomously while contributing to a multidisciplinary rehabilitation service.
Key Responsibilities

- Undertake independent occupational therapy assessment, treatment, and discharge planning for patients within the community rehabilitation service.
- Plan, manage, and prioritise your own caseload in line with service needs.
- Develop and implement individualised treatment and rehabilitation plans.
- Use recognised outcome measures and sound clinical reasoning to guide interventions and monitor progress.
- Support patients to improve functional independence following illness, injury, or hospital admission.
- Work collaboratively within a multidisciplinary team to ensure coordinated rehabilitation and safe discharge.
- Maintain accurate and timely clinical documentation in line with Trust and HCPC standards.
About You
You are a proactive and capable Occupational Therapist with post-qualification experience, ideally within community or rehabilitation services. You bring strong assessment and clinical reasoning skills and are confident managing a varied caseload independently. Your flexible and person-centred approach supports effective rehabilitation and positive outcomes for patients living in the community.
Requirements

- Degree or diploma in Occupational Therapy with current HCPC registration.
- Post-qualification experience working as an Occupational Therapist within the NHS.
- Experience within community rehabilitation services is desirable.
- Confidence in autonomous caseload management and treatment planning.
- Strong communication, documentation, and organisational skills.
- Car driver is preferable due to the community-based nature of the role.
