About the Role
Mid Cheshire Hospitals NHS Foundation Trust is seeking a skilled Band 6 Community Occupational Therapist or Physiotherapist to support community rehabilitation services, including Pathway 2 rehab. This role is suitable for either profession—or a clinician with experience across both disciplines—providing autonomous assessment and rehabilitation for adults in the community.
Main Responsibilities
- Deliver community-based rehabilitation intervention as part of the Pathway 2 service.
- Undertake holistic assessments and create tailored treatment or therapy plans to support recovery and maximise independence.
- Manage a varied community caseload, visiting patients in their homes or community settings where required.
- Contribute to safe discharge planning and ongoing rehabilitation following hospital admission.
- Work collaboratively with the multidisciplinary team to ensure coordinated care delivery.
- Maintain accurate clinical documentation in line with service and professional standards.
Skills & Experience Required
- HCPC-registered Occupational Therapist or Physiotherapist.
- Experience in community rehabilitation and/or Pathway 2 rehab is essential.
- Strong assessment, clinical reasoning and caseload management skills.
- Experience working autonomously in community settings.
- Excellent communication and MDT collaboration abilities.
Working Hours
- Monday to Friday, 09:00 – 17:00.
