About the Role
The London Borough of Hackney is seeking a skilled Occupational Therapist to deliver high-quality, community-based assessments and interventions for adults with permanent, complex or substantial disabilities. This role focuses on promoting independence, reducing long-term support needs and ensuring individuals receive the right environmental and care-related solutions to enable safe and autonomous living.
Main Responsibilities
- Carry out comprehensive community assessments for adults with permanent or complex disabilities, identifying solutions that prevent the need for ongoing services.
- Conduct assessments under the Care Act and the Housing Grants, Construction and Regeneration Act, ensuring environmental changes are reflected accurately in care planning.
- Work collaboratively as part of a multidisciplinary team to review existing care packages, exploring opportunities to enhance independence and reduce reliance on long-term services.
- Supervise Occupational Therapy Assistants and OT students, supporting high-quality practice and development.
- Contribute to OT and Sensory Service functions aligned with Adult Social Care’s broader vision and ambitions.
- Work in partnership with service users, carers, and a range of professionals to maximise independence and deliver person-centred outcomes.
- Review support packages, equipment and adaptations to ensure they remain appropriate and effective.
- Maintain accurate and timely case notes using approved recording systems.
- Assess and prescribe Telecare and assistive technologies to promote safety, independence and self-management.
Requirements
- HCPC-registered Occupational Therapist.
- Experience within adult social care, housing, equipment or adaptations services.
- Sound understanding of relevant legislation including the Care Act and Housing legislation.
- Strong assessment, clinical reasoning and problem-solving skills.
- Ability to supervise assistants and students effectively.
- Excellent communication, time management and partnership working skills.
