About the Role
Salisbury District Hospital is seeking a proactive and experienced Band 6 Occupational Therapist to join the Early Supportive Discharge team. This role is essential to supporting safe, timely and effective patient discharge, bridging both acute and community settings to ensure continuity of care.
Main Responsibilities
- Carry out holistic occupational therapy assessments for patients preparing for early discharge.
- Assess care needs, identify barriers to discharge and provide solutions through advice, equipment and rehabilitation.
- Order and arrange timely provision of appropriate equipment.
- Identify patient goals and create clear, achievable treatment and support plans.
- Work collaboratively within an MDT and across health and social care providers.
- Provide support across both acute and community components of the service.
- Maintain accurate documentation in line with HCPC and NHS requirements.
- Adopt a flexible, proactive approach with a strong “can-do” attitude.
Requirements
- HCPC-registered Occupational Therapist.
- Experience in both acute and community working.
- Strong assessment and clinical reasoning skills.
- Experience in equipment prescription and discharge planning.
- Strong MDT working skills and ability to work across providers.
- Positive, adaptable and proactive professional approach.
Working Hours
- 08:00 – 17:00.
- Weekend work may be required.
