About the Role
An excellent opportunity has arisen for an experienced Band 6 Community Occupational Therapist to join the Community Rehab team covering Hull and the East Riding. This role requires a confident and autonomous practitioner with strong community-based clinical skills, supporting adults with a wide range of functional, neurological and rehabilitation needs.
Main Responsibilities
- Deliver comprehensive occupational therapy assessments and interventions within patients’ homes and community settings.
- Work autonomously, managing a varied caseload across Hull and the wider East Riding area.
- Support individuals with a range of conditions including falls, frailty, neurological conditions and postural management needs.
- Develop person-centred rehabilitation plans that promote safety, independence and quality of life.
- Collaborate with MDT colleagues to ensure effective, coordinated care.
- Maintain accurate clinical documentation in line with service and HCPC standards.
Requirements
- HCPC registered Occupational Therapist.
- Experience working in community settings, ideally community rehab.
- Neuro experience preferred.
- Strong assessment, clinical reasoning and caseload management skills.
- Reliable, autonomous practitioner able to work across a wide geographical area.
- Full UK driving licence and ability to travel across Hull/East Riding.
- S1 experience desirable but not essential.
Working Hours
- Monday to Friday, 08:30 – 16:30 (flexibility available).
