Community Occupational Therapist – Band 6 (Locum)

 

Location: City Health Care Partnership CIC

Area: East Riding of Yorkshire NHS

Rate: £Negotiable

Contract: Locum, Ongoing

 

About the Role

 

An excellent opportunity has arisen for an experienced Band 6 Community Occupational Therapist to join the Community Rehab team covering Hull and the East Riding. This role requires a confident and autonomous practitioner with strong community-based clinical skills, supporting adults with a wide range of functional, neurological and rehabilitation needs.

 

Main Responsibilities

 

  • Deliver comprehensive occupational therapy assessments and interventions within patients’ homes and community settings.
  • Work autonomously, managing a varied caseload across Hull and the wider East Riding area.
  • Support individuals with a range of conditions including falls, frailty, neurological conditions and postural management needs.
  • Develop person-centred rehabilitation plans that promote safety, independence and quality of life.
  • Collaborate with MDT colleagues to ensure effective, coordinated care.
  • Maintain accurate clinical documentation in line with service and HCPC standards.

 

Requirements

 

  • HCPC registered Occupational Therapist.
  • Experience working in community settings, ideally community rehab.
  • Neuro experience preferred.
  • Strong assessment, clinical reasoning and caseload management skills.
  • Reliable, autonomous practitioner able to work across a wide geographical area.
  • Full UK driving licence and ability to travel across Hull/East Riding.
  • S1 experience desirable but not essential.

 

Working Hours

 

  • Monday to Friday, 08:30 – 16:30 (flexibility available).
