An exciting opportunity has arisen for an experienced and compassionate Occupational Therapist to join the Disabilities Early Intervention & Prevention Services with Conwy County Borough Council.  
This part-time role offers a chance to make a meaningful difference in the lives of individuals and families across North Wales by delivering outcome-focused interventions that promote resilience, independence, and well-being.  

The Early Intervention & Prevention (EIP) Team provides short-term, person-centred support to empower people with disabilities and their families to manage their daily lives more effectively. You will play a key role in assessing needs, providing direct occupational therapy, supporting carers, and promoting independence through a strengths-based approach.  

Key Responsibilities

 

  • Deliver a direct Occupational Therapy service to individuals with disabilities, focusing on enhancing independence and daily functioning.
  • Work collaboratively with parents, carers, and families to build resilience and develop self-management strategies.
  • Undertake screening assessments and make recommendations for appropriate interventions and support.
  • Contribute to safeguarding work, carrying out activity and referrals as required.
  • Ensure individuals and families are actively involved in decision-making and goal setting to achieve positive, person-centred outcomes.
  • Promote early intervention and prevention principles to reduce dependency on long-term services.
  • Provide advice, training, and guidance to carers, families, and other professionals.
  • Maintain accurate and up-to-date records, using technology and software systems efficiently.
  • Participate in multi-disciplinary meetings and contribute to joint assessments and support plans.

About You

 
You are a dedicated and self-motivated Occupational Therapist who is passionate about promoting independence and supporting people with disabilities to live full, meaningful lives. You have a person-centred approach and the ability to build positive, empowering relationships with individuals and their families.  

You bring excellent assessment and problem-solving skills, along with the ability to work both autonomously and collaboratively within a multidisciplinary team.  

Requirements

 

  • BSc (Hons) Degree in Occupational Therapy (or equivalent qualification).
  • HCPC Registration as a qualified Occupational Therapist.
  • Minimum of three years post-qualification experience within social care or community disability settings.
  • Experience working with individuals with disabilities and their families.
  • Proven ability to conduct comprehensive functional assessments and make informed clinical recommendations.
  • Excellent communication, teamwork, and interpersonal skills.
  • Experience in manual handling assessment and training.
  • Strong IT and case management software skills.
  • Knowledge of safeguarding processes and relevant legislation.
  • Ability to prioritise workload and manage competing demands effectively.

Contract & Working Pattern

 

  • Hours: 23 hours per week
  • Days: Monday to Friday
  • Working Pattern: Part-time (flexible arrangements considered)
  • Contract: Locum – initial 12 weeks, with potential extension
  • Location: Conway Road, Colwyn Bay
  • Salary: £Negotiable (Umbrella rate: £26.22 per hour)

The Disabilities Early Intervention & Prevention Team is dedicated to supporting people with disabilities and their families to live independently and with confidence. The team provides short-term, outcome-focused interventions that promote self-reliance, improve quality of life, and prevent the escalation of care needs.
